You can also send invitations and collect donations online.

  1. Use this donation form
    • Send to supporters or have on hand the big day.
    • Each supporter who wishes to donate should fill out a form or donate online.
    • Checks should be made out to the Alzheimer's Association.
    • Collect all the forms and checks.


  2. Fill out the event report form
    After your event, fill out the host report form.

  3. Mail in donations within 30 days
    • Use one envelope.
    • Include (1) the host report form, (2) all donation forms and (3) all checks.
    • Send in one envelope to:

      Chicago, IL 60690

The Alzheimer's Association will send each donor an acknowledgment of their gift.
