		If you had a party... Use this donation form Send to your guests or have on hand. Each guest who wishes to donate should fill out a form or donate online. Checks should be made out to the Alzheimer's Association. Collect all the forms and checks.
		Fill out the host report form After your event, fill out the host report form. Mail in donations within 30 days Use one envelope. Include (1) the host report form, (2) all donation forms and (3) all checks. Send in one envelope to: Host an Event Alzheimer's Association P.O. Box 2940 Chicago, IL 60690
	The Alzheimer's Association will send each donor an acknowledgment of their gift.
